This is where it becomes hairy: not difficult, just hairy.
We will assume that you are registering your web site for the first time. (we'll go through registering changes later.) There are two different forms that can be used for online works: the short Form TX and the long Form TX. Both can be printed or downloaded from http://www.copyright.gov/forms. Either way, your form must be submitted in hard copy.
You can download a PDF version of Form TX from http://www.copyright.gov/forms/formtxi.pdf or the short version from http://www.copyright.gov/forms/formtxs.pdf
Observe the form reverently as with it you will be interacting with the government. Do not fear it for this reason. The Copyright Office is not malignant like some we could name.
You can type on the form, or print on it by hand; if you do, you must use black ink. The instructions remind you to be "clear and legible" as your certificate will be copied from the form. But you may find it easier to open the PDF version in Adobe Acrobat Reader and fill it in there before printing. (Click on the checkboxes to add or delete a check mark; adjust the font size to make it as legible as possible.) Note that you have to print the form in two-sided fashion.
(You could save the forms permanently as there's a good chance you'll be repeating this exercise. But check now and then to see if they have changed; look at the fine print at the bottom to see the revision and form numbers.)
